UPSECTS Scheme Registration 2021 / Login: The Uttar Pradesh government is inviting online applications for the State Health Card under the State Employees Cashless Treatment Scheme. To get the state health card under the scheme, the UP SECTS scheme online application form can be filled on the official website of the scheme up.hp.in.
Under the State Employees Cashless Treatment Scheme, the government provides cashless treatment to its employees in selected health and medical centers across the state. The complete procedure to apply online for UP State Health Card is given below.
Online application for UP State Health Card
Phase 1: Interested state government employees will have to visit the official website of this scheme http://up-health.in/en/
step 2: Under the main menu, move your mouse cursor over “Citizen’s cornerClick on the “tab” and then “Citizen loginClick the link or directly http://up-health.in/online
step 3: Registered employees / pensioners can then login, while new users can apply for the UP State Health Card by clicking on the “New Registration” link.
step 4: direct link – http://up-health.in/online/Account/Registration. An application form will open as below, fill all the required details in the application form and click on the SUBMIT button at the bottom of the page.
step 4: Once you click on the SUBMIT button, your application will be sent to the department and on the next screen you will be provided with a registration number.

Applications for the state health card can also be submitted through offline mode. The application form can be downloaded using the link given below and submitted to the DDO’s office. You can later edit or print your submitted application form by entering your Aadhaar number through “Edit application form“And”Print the application form“the option.
Check status of state health card application
After submitting the application, applicants can also check the status of their state health card application. To check the status, applicants have to enter their Aadhaar number or registration number at the time of registration.
